Identifikace zařízení, ve kterém byla zkouška provedena – název, adresa, IČO osoby, která provedla zkoušku funkčnosti části vozidla, příp. opravu části vozidla. Uvede se, bylo-li přiděleno.
Jméno a kontaktní údaje autorizovaného znalce nebo mechanika vozidla, který zkoušku prováděl – autorizovaným znalcem se rozumí osoba oprávněná vykonávat znaleckou činnost, samostatný likvidátor pojistných událostí nebo osoba s odpovídající kvalifikací v zemi odeslání. Mechanikem vozidla se rozumí osoba oprávněná k podnikání v oboru opravy a údržby motorových vozidel nebo osoba s odpovídající kvalifikací v zemi odeslání. Jedná se o osoby podle § 112 odst. 4 zákona.

Holder of the vehicle – the details of the vehicle owner before the transboundary shipment shall be provided; if the owner is a legal entity, the name of the company and the ID number shall be stated. In the case the holder is not the same person as owner, it is necessary to prove with the power of attorney the owner of the vehicle.
Specification of the vehicle – vehicle identification data (type, mark, model, VIN code, mileage on the clock, estimated market value) must be completed; the mileage on the clock must correspond to the current condition of the vehicle during transport.
The testing procedure for the vehicle – the procedure by which the test was performed on the vehicle (check of identification of the vehicle, braking systém, vehicle control system, chassis and other parts of the vehicle), description of the damage found, estimated repair costs, where appropriate, a description of the repair, if any, and other relevant information.
Identification of the facility, where the vehicle was tested – name, address, identification number of the person, who performed the technical test of the vehicle.
Name and contact details of the authorized expert or vehicle mechanic, who has tested the vehicle – Authorized expert means a person authorized to carry out expert activities, an independent claims adjuster or a person with appropriate qualifications in the country of destination. Vehicle mechanic means a person authorized to do business in the field of repair and maintenance of motor vehicles or a person with appropriate qualification in the country of destination. It is a person according to § 112 paragraph 4 of the Act no. 542/2020 Coll., on end-of-life products.
Declaration – authorized expert or mechanic will indicate a suitable variant and, if the vehicle requires repair, state the estimated price of the repair in CZK or EUR.

Vehicle part specification – fill in the official or utility name of the vehicle part, the designation of the vehicle part, the specification of the vehicle from which the vehicle part originates, including VIN identification numbers, and the estimated current market value of the vehicle part.
The testing procedure for the vehicle part – the procedure by which the test was performed on the vehicle part (specifically according to the transported part of the vehicle), the resulting test values, measurements, description of the repair of the vehicle part, if any, etc.
